IMPORTANT!  Your open enrollment notice will not be generated for this Open Enrollment Period.  Please read the following OE Notice:  OE General Notice - November 4th

The Board of Trustees has added a new PPO medical plan option.  The current 90/10 PPO plan will continue and will be administered only by HMA (Health Management Association).  A new 80/20 PPO plan with lower premiums will be administered by HMSA.
Note:  “90/10″ or “80/20″ refers to most benefits; see Reference Guide for Actives or Guide to Benefits for details.

ØIf you are currently enrolled in the EUTF PPO Plan administered by either HMA or HMSA and:

1. Wish to keep your same plan benefits of the 90/10 PPO,
üDo nothing and you will be enrolled in the EUTF 90/10 PPO Plan administered only by HMA effective January 1, 2010 through June 30, 2010.
OR
2. Want to change your plan benefits to 80/20 PPO,
üYou must submit the Form EC-1 to your personnel/employer by November 30, 2009.  Effective January 1, 2010, you will be enrolled in the EUTF 80/20 PPO Plan administered only by HMSA.
